What Is Bipolar Disorder?
Bipolar disorder is a group of mental health conditions that cause dramatic shifts in mood, ranging from emotional highs (mania or hypomania) to lows (depression). It was previously called manic-depressive illness and affects both emotional well-being and behavior.
There are two primary types of bipolar disorder:
-
Bipolar I Disorder – Involves at least one manic episode lasting a week or more, often followed by depressive or hypomanic episodes.
-
Bipolar II Disorder – Involves one or more major depressive episodes and at least one hypomanic episode, without ever experiencing full manic episodes.
A third, milder form—cyclothymia—involves less severe mood swings but still requires clinical care.
What are manic and depressive episodes in bipolar disorder?
Manic or Hypomanic Episodes
These "high" phases may include:
-
Elevated or irritable mood
-
Racing thoughts or rapid speech
-
Increased energy or activity
-
Decreased need for sleep
-
Impulsivity or risky behavior (e.g., overspending, reckless driving)
-
Grandiosity or inflated self-esteem
Mania lasts at least one week; hypomania is similar but shorter and less intense, typically lasting at least four days.
Depressive Episodes
These "low" phases may include:
-
Prolonged sadness or hopelessness
-
Loss of interest in activities
-
Fatigue or low energy
-
Trouble concentrating or making decisions
-
Sleep and appetite disturbances
-
Feelings of worthlessness
-
Thoughts of death or suicide
Depressive episodes typically last at least two weeks and can be deeply impairing if untreated.
Which Type Is More Severe?
Bipolar I disorder is generally considered more severe due to the intensity of manic episodes, which can lead to hospitalization. However, bipolar II disorder can also be debilitating particularly because depressive episodes are often more prolonged. No matter the type, both require medical care and respond well to treatment.
How Is Bipolar Disorder Treated?
At Stepping Stone, we create a customized care plan to address your unique experience with bipolar disorder. Treatment may include:
Medication Management
Mood stabilizers are often the foundation of bipolar treatment. These medications help regulate brain chemistry and prevent extreme highs and lows. Depending on your symptoms, antidepressants, antipsychotics, or anti-anxiety medications may also be prescribed and closely monitored.
Psychotherapy
Also known as talk therapy, psychotherapy helps you recognize triggers, manage stress, and develop strategies to maintain emotional balance. Cognitive Behavioral Therapy (CBT) and other evidence-based approaches are used to build long-term coping skills and reduce relapse.
